Client location and schedule hours:
Client located in Belleview Area. Schedule is T-W-Th 7 am to 1 pm.
Job Responsibilities:
Goals & Objectives: Client mainly needs companionship. Observe and encourage her to use her walker on every transfer. Go on walks with her daily In AM) about a mile around her neighborhood observing her gait for unsteadiness. Assist with fastening of clothing.

Goals & Objectives: Client mainly needs companionship. Observe and encourage her to use her walker on every transfer. Go on walks with her daily In AM) about a mile around her neighborhood observing her gait for unsteadiness. Assist with fastening of clothing.
ADL's: Mobility: She is supposed to use a walker or at least get up slowly to help avoid dizziness. She walks fast and may need to be reminded to slow down on walks. Assist her on walks to observe the curbs and any uneven ground.
Toileting: She is independent but admits she wears Poise pads. Would be helpful to ask if she needs to go every 2 hours if she's not gone.
Bathing/Dressing/Grooming: She dresses in matching clothes and appears very well kempt. She is very high functioning, independent in showering, Offer standby assist when she wants to get a shower. She chooses her own clothes and puts them on (may need help with fastening clothes).
Eating: Client prepares her own meals weekly and puts them in containers for her to eat each day. She is a very healthy eater and favors salads and lots of veggies. She reports to not be on any special diets. She feeds herself and usually cleans up the kitchen herself afterwards. She may offer CG a meal randomly but likes it if visitors eat with her if they are there at her mealtime.
Communication: Client has a serious hearing loss-speak loud and clear. Sue communicates very well, is articulate and can make her own needs known. Client reported that reading books is her favorite activity & she watches the news sometimes. She was very talkative and you can see that client is correct in that she needs someone to talk to.
Qualifications:
Minimum of 1 year experience as an in-home Caregiver, CNA or related experiences with a loved one of family member
Ability to effectively communicate through written and verbal language
Valid Driver's License and adequate auto insurance
Consent to criminal, elder abuse, motor vehicle and sex offender background checks
Consent to drug and alcohol testing
TB test
Compensation: $15.00 - $17.00 per hour
